What Causes Gaps Between Front Teeth? How Can They Be Closed?
A space between the front teeth is called a diastema in dentistry. It is most often noticed between the two upper front teeth. Although it may seem like a purely aesthetic concern, closing the space without identifying the underlying cause may not provide a stable long-term result.
The gap may be related to tooth size, jaw structure, the upper lip frenulum, gum health, oral habits or missing teeth. Therefore, the treatment plan is not the same for every patient.
What Is a Gap Between Front Teeth (Diastema)?
Diastema means that there is more space than usual between two teeth. When it appears in the front area, it can directly affect smile aesthetics. In childhood, gaps between front teeth are often a natural part of development and may decrease as permanent teeth erupt.
In adults, however, a newly formed or gradually widening gap should be evaluated more carefully. If there is tooth mobility, gum recession, bleeding or a change in the bite, the gum and bone support should be examined first.
Why Do Gaps Form Between Front Teeth?
A gap between front teeth may have more than one cause. Common reasons include:
- Mismatch between tooth and jaw size: If the teeth are relatively small for the jaw, spaces may appear between them.
- A thick or low-attached lip frenulum: The tissue between the upper lip and gum can separate the two front teeth.
- Missing or small lateral incisors: Congenitally small or missing side teeth can increase spacing in the front area.
- Tongue thrusting and habits: Pressure from the tongue during swallowing, thumb sucking or biting objects can affect tooth position.
- Gum disease: Loss of supporting tissues may cause teeth to shift and spaces to open later in life.
- Orthodontic bite problems: Tooth alignment and jaw relationship may contribute to spacing.
When Is a Dental Examination Needed?
If the gap between the front teeth has been stable for a long time and only causes aesthetic concern, treatment is usually planned from a smile design perspective. However, if the gap is new or has been increasing over time, a dental examination should not be delayed.
The following signs may indicate an underlying gum or bite problem:
- Bleeding or swelling of the gums
- A feeling of tooth mobility
- Gum recession
- Front teeth moving forward
- A change in the bite
- A gap becoming noticeable in a short time
How Can Gaps Between Front Teeth Be Closed?
The treatment option depends on the cause and size of the gap, tooth shape and the patient’s expectations. Common methods include:
Orthodontic treatment
If there are multiple spaces, a bite problem or a need to reposition the teeth, braces or clear aligners may be preferred. This approach moves the teeth into a natural position and is a strong option for structural spacing.
Bonding
For small to moderate gaps, the side surfaces of the teeth can be reshaped with tooth-colored composite resin. It is usually a quick procedure and requires minimal intervention to tooth structure.
Porcelain laminate veneers
Veneers may be considered when tooth shape, color and proportions need to be improved together. They can be suitable when teeth are small or smile design is planned.
Frenulum treatment
If a thick frenulum is the cause, a frenectomy may be planned when necessary. It is often evaluated together with orthodontic or aesthetic treatment.
Gum treatment
If the gap is related to gum disease, periodontal treatment should be performed first. Closing the space cosmetically without stabilizing the supporting tissues may not be reliable long term.
Can the Gap Open Again After Treatment?
Determining the cause correctly is essential to prevent the gap from reopening. After orthodontic treatment, retainers or fixed retention wires may be used. After bonding or veneers, habits such as teeth clenching, nail biting or biting pens should be controlled.
Treatment of gaps between front teeth is not only about closing the space. Tooth shape, gum health, bite relationship and smile aesthetics should be evaluated together. The most suitable method is determined after a detailed examination and an individual treatment plan.
