Why Does Chewing on One Side Occur?

The habit of chewing on one side often develops unnoticed. Both mechanical and functional reasons may underlie it:

  • Feeling more comfortable chewing on the same side

  • Broken tooth, gum recession, or implant area
  • Orthodontic malocclusions (bite problems)

On the side that bears the constant load, enamel wears down more quickly. When the load balance is disrupted:

  • Chewing surfaces become flattened

  • Cracks may develop in some teeth

This is especially noticeable when consuming hard foods; even slight pressure can cause pain.

2. Gum Problems
Brushing on the active side is usually more vigorous, which can lead to:

  • Morning jaw fatigue
  • Pain around the head, neck, and ears

Patients often do not notice this; complaints may simply present as “my jaw feels off.”

Can Facial and Muscle Asymmetry Develop?

Yes. Not only the teeth but also the facial muscles are affected by this habit. The masseter muscle on the active side works more and strengthens, while the other side weakens, disrupting facial symmetry.

In some patients, this can result in:

  • Unilateral cheek fullness
  • Asymmetry in facial lines
  • Disturbance in smile symmetry

In such cases, not only the jaw but also facial aesthetics are impacted.

What Should the Treatment Approach Be?

The goal of treatment is to correct intraoral causes and balance the chewing habit.

Key points assessed during examination:

  • Is there decay, a root canal, filling, or sensitivity on the active side?
  • Why hasn’t the other side been used until now?
  • Is the jaw joint stable?
  • Is the occlusion correct?
  • Is the gum condition balanced?

Once the problem is identified, the treatment plan is individualized. If necessary, interventions may include:

  • Filling or fractured tooth restoration
  • Orthodontic evaluation
  • Gum treatment
  • Muscle relaxation exercises
  • Jaw joint support protocols

Unilateral chewing, if unnoticed, can develop into significant oral and facial asymmetry over the years. The solution is often simpler than expected: awareness, balance, and proper intervention. In mild habits, improvement can be seen within a few weeks, while jaw joint or occlusion issues may require longer monitoring. Regular follow-up and compliance are crucial.
