Tooth nerves are located in the pulp tissue at the center of the tooth and are the essential structures that give the tooth its vitality. These nerves do more than transmit pain; they also support the tooth’s nutrition, defense, and tissue regeneration. However, in some cases, this living tissue is damaged and the tooth nerves die (necrosis). The process usually progresses silently; most people notice it only when pain or discoloration appears. If detected late, the infection can spread to the surrounding bone and require more advanced treatment.
Why Do Tooth Nerves Die?
Nerve death usually develops as a result of a long-standing, untreated problem. The most common cause is deep cavities; over time, bacteria reach the pulp tissue, causing inflammation and cell death. Trauma is another significant factor. Sports injuries, impacts, or biting a hard object can disrupt blood flow to the tooth, causing the nerve to gradually die. In some patients, old large fillings or excessive teeth grinding (bruxism) can also fatigue the nerve tissue and lead to gradual damage.
Sometimes nerve death progresses without any symptoms. That’s why regular check-ups are especially important for people with large fillings or old restorations.
Symptoms: What Do You Feel When a Tooth Nerve Begins to Die?
Not every nerve death causes pain, which makes diagnosis more difficult. However, common symptoms include:
- Pain triggered by cold or hot stimuli
- Throbbing that worsens at night
- Sensitivity while chewing
- Darkening or graying of the tooth
- Pimple-like fistula formation near the gum line
- Bad taste or odor in the mouth
In some cases, when the nerve is completely dead, the pain disappears and the patient may think the tooth is healed. In reality, this is the stage when bacteria silently spread to the bone. Therefore, canceling treatment just because “the pain is gone” can lead to serious complications.
How Is Tooth Nerve (Root Canal) Treatment Performed?
In such cases, the procedure performed is a root canal treatment. The goal is to remove the damaged nerve and bacteria from the root canals, disinfect the canal system, and seal it with a special filling material. Modern techniques using rotary instruments, microscope assistance, ultrasonic irrigation systems, and biocompatible filling materials achieve very high success rates.
In some teeth, antibiotics may be needed in addition to the procedure, especially if there is widespread bone infection. However, not every nerve death requires antibiotics; the decision depends on the dentist’s evaluation.
After root canal treatment, the tooth may become more fragile, so a crown is often recommended. This ensures that the tooth maintains its function for many years.
What Happens If Treatment Is Delayed?
If treatment is delayed:
- An abscess may form
- Bone loss can develop
- Severe pain and swelling may occur
- The tooth may be lost
Additionally, an advanced infection can spread to the facial area, creating situations that require emergency intervention. For this reason, patients who suspect nerve death should have a dental examination without delay.
Saving the Tooth or Pulling It?
In modern dentistry, the priority is always to keep the tooth in the mouth. Root canal treatment, with proper planning and restoration, can provide very long-lasting results. Extraction is only preferred when the tooth cannot be saved, and afterward, options such as implants, bridges, or dentures are considered.
A healthy root canal treatment not only relieves pain but also preserves the tooth’s function for years. Regular check-ups, early treatment of cavities, and prompt intervention in dental trauma significantly reduce the risk of nerve death. Remember, a tooth can be damaged silently; therefore, seeing your dentist before pain starts is the best approach.
