Baby bottle tooth decay is a special type of decay that begins especially in the upper front teeth of infants and young children and progresses rapidly. It usually occurs when a child drinks milk, fruit juice, or sweetened formula from a bottle while sleeping at night. Since saliva production decreases during sleep, the mouth cannot clean itself, and the teeth remain in contact with sugar for a long time. Many families think that “milk is natural and harmless,” but even milk can cause decay due to the lactose it contains. This condition is not just an aesthetic problem; it can lead to pain, feeding difficulties, delays in speech development, and even crooked teeth in the future. Therefore, recognizing baby bottle tooth decay early and taking the correct precautions is extremely important.
Symptoms of Baby Bottle Tooth Decay
Baby bottle tooth decay usually begins with white chalky spots. If these spots are ignored, they quickly turn into brown-black cavities, followed by areas of breakage and collapse. In later stages, the child may experience sensitivity to hot and cold, reluctance to eat, irritability, and bad breath.
Many parents overlook these changes by saying, “the teething period is normal.” However, cavities detected at an early stage can be treated with much simpler methods.
Why Does Baby Bottle Tooth Decay Occur?
The main cause of this type of decay is sugary drinks remaining on the teeth for a long time. The most common mistakes are:
- Putting the child to sleep with a bottle at night (milk, fruit juice, formula)
- Not cleaning the teeth after feeding
- Applying honey / molasses / sugar to the pacifier
- Sharing the same spoon or putting the pacifier in the mouth and then giving it to the child (bacterial transmission)
The mindset of “it’s okay, they’re just baby teeth” can unfortunately lead to early tooth loss.
How Is Baby Bottle Tooth Decay Treated?
Treatment varies depending on the severity of the decay. In the early stage, tooth structure can be strengthened with fluoride applications and remineralization agents. In more advanced stages, fillings, pediatric root canal treatment, or stainless steel crowns may be required. In cases where the tooth cannot be saved, extraction is preferred, and a space maintainer may be applied if necessary.
The goal is not only to stop the decay, but also to protect the child’s speech, chewing, and aesthetic development. Preventing baby bottle tooth decay is much easier than treating it. Establishing a cleaning routine as soon as the teeth begin to erupt is the most critical point. Night feeding habits should be gradually reduced, a transition to water should be encouraged, and after the age of 1, the use of a cup instead of a bottle should be promoted.
In addition, it is not sufficient for parents to say “the child brushed their teeth”; young children cannot brush effectively due to undeveloped motor skills. The final brushing must always be completed by a parent.
Golden rule: Teeth must always be cleaned after eating; there should be no “sleep after eating.”
