The black spaces seen between teeth, also known as interdental papilla loss, can be both aesthetically displeasing and functionally problematic. These areas are commonly referred to as “black triangles.” When the papilla loses its function, it not only negatively affects the smile line but also allows food debris to accumulate, reducing patients’ daily comfort.

In modern dentistry, the goal is not only to make the teeth appear white and aligned; the harmony between the gums and teeth and the fullness of the papilla are fundamental elements of a healthy and youthful smile.

What Is the Papilla and Why Is It Important?

The papilla is the small triangular gum tissue located between two teeth.
This structure creates a natural aesthetic and also prevents food from filling the gaps.

When the papilla is healthy:

  • Teeth appear natural, tight, and esthetically pleasing
  • Gums look younger and fuller
  • Risk of bad breath and plaque accumulation decreases

Papilla recession can create an “aged” appearance in the smile.

Common Causes of Interdental Papilla Loss

1. Gum Disease and Loss of Bone Support
When the bone supporting the tooth recedes, the papilla also loses support. This process often progresses unnoticed, and gaps may suddenly appear more prominent.

2. Aggressive Brushing and Gum Trauma
Using a hard toothbrush or brushing with incorrect technique can wear down the papilla tissue. Patients often press harder for a “cleaner” feel, which can lead to tissue loss.

3. Post-Orthodontic Gap Visibility
When teeth are aligned, gaps between them may become visible. Orthodontic treatment is not faulty; the new position of the teeth can make existing gaps more noticeable.

4. Aging
Over time, tissue volume decreases and connective tissues lose elasticity. Therefore, papilla loss can also be associated with aging.

5. Tooth Shape and Interdental Distance
Papilla loss develops more easily in triangular-shaped teeth. When the contact point of the teeth is lower, support for the papilla is reduced.

How Is Papilla Loss Treated?

Treatment is planned individually, as there is no single solution; the cause and extent of papilla loss vary for each patient.

1. Soft Tissue Grafts
In some cases, tissue taken from the palate or using ready-made biomaterials is used to support the papilla area and restore volume.

2. Hyaluronic Acid or Biologic Fillers
Biocompatible fillers are injected into the papilla area to softly fill the gap.
Provides immediate aesthetic effect and has a quick recovery process.

3. Orthodontic Micro-Adjustments
The contact points of teeth can be realigned to create appropriate space for the papilla tissue.

4. Restorative Adjustments (Veneer / Composite Recontouring)
The shape of the teeth is corrected to make gaps more aesthetic.
Proper design visually restores the appearance of the papilla.

Correct Approaches That Can Be Applied at Home

This condition cannot be completely corrected at home, but supporting the tissue’s recovery is possible:

  • Avoid hard brushes; choose a soft-bristled brush
  • Learn the correct brushing technique (sweeping motion without pressure)
  • Use dental floss and interdental brushes carefully
  • Prevent dry mouth
  • Have regular professional cleanings

Incorrect practices — especially grinding, using a hard floss, or herbal mixtures recommended online — can worsen the condition.

Does Papilla Loss Heal on Its Own?

In mild cases, supportive treatments and proper care can partially restore the tissue.
In moderate and advanced cases, clinical interventions are required. Early awareness is the greatest advantage.
