Gastroesophageal reflux disease (GERD) is not only a condition affecting the stomach; it can also directly impact oral and dental health. During reflux, stomach acid reaches the mouth through the esophagus, leading to significant oral problems such as bad breath, enamel erosion, and tooth sensitivity. Many patients ask, “I have bad breath, but my teeth are clean—why?” The answer is often silent reflux. The acidic environment created by reflux in the mouth gradually thins the enamel and weakens the teeth’s protective barrier over time. Tooth wear due to reflux is especially common in individuals who experience a bitter taste, burning sensation, or dry throat in the morning.

How Does Reflux Cause Tooth Wear?

Stomach acid has a very low pH, and enamel cannot withstand such strong acid for long periods. When acid reaches the oral environment, the enamel softens, thins, and gradually begins to wear away. This process can be especially noticeable on the inner surfaces of the front teeth.

The risk increases in:

  • Individuals experiencing nighttime reflux attacks
  • Those who sleep with their mouth open
  • People with a history of frequent vomiting (pregnancy, reflux episodes, or after intense exercise when stomach acid rises)

As enamel thins, teeth begin to appear yellow, change shape, and break more easily. At the same time, sensitivity to hot and cold becomes more pronounced.

Does Reflux Cause Bad Breath?

Yes. Reflux is one of the main causes of persistent bad breath that does not go away even with tooth brushing.
The backflow of stomach contents into the throat and mouth causes the smell of acid, bile, and undigested food to be noticeable in the mouth. This also increases bacterial colonies on the teeth, making the odor more pronounced.

If you experience both a burning sensation in the throat and bad breath, the issue may not be related to your teeth alone.

How to Distinguish Between Reflux-Related Bad Breath and Tooth-Related Odor?

If the following signs are present, doctors may request a gastroenterological evaluation:

  • The odor persists despite treatment for tartar and cavities
  • It is more noticeable in the mornings
  • Accompanying sour taste and burning sensation in the throat
  • Symptoms worsen, especially when lying down

Tooth Erosion Symptoms

Tooth wear caused by reflux manifests with the following symptoms:

  • Dullness on the tooth surface, loss of shine
  • Yellowing of teeth
  • Sensitivity to hot and cold
  • Enamel cracks
  • Minor aches while eating
  • Thinning and transparency at the edges of teeth

As this condition progresses, not only aesthetics but also function and comfort are affected.

How Is Tooth Wear Caused by Reflux Treated?

Treatment involves both controlling stomach acid and protecting the oral tissues.

Preferred dental approaches include:

  • Remineralizing agents that strengthen tooth tissue
  • Sensitivity-relieving applications
  • Composite restorations or porcelain repairs when necessary
  • Special fluoride products that protect enamel
  • Rinsing the mouth instead of brushing immediately after acid exposure

Brushing immediately after acid contact is a mistake, as it can accelerate the wear of softened enamel.
