Tooth erosion is the thinning and weakening of the tooth surface caused by the chemical dissolution of enamel tissue due to acid exposure. While bacteria play a role in cavities, the main problem in erosion is acid attack.
Over time, this condition manifests as sensitivity, yellowing, enamel loss, surface roughness, and difficulty chewing. If not detected early, the dentin layer becomes exposed, leading to faster tissue loss.

Today, tooth erosion is seen in all age groups, especially due to acidic beverage consumption, acid reflux, and poor dietary habits.

Common Causes of Tooth Erosion

Acidic Drinks and Foods
Carbonated drinks, fruit juices, sports drinks, and lemon water can dissolve minerals on the tooth surface.
The longer the acid contact, the higher the risk of erosion.

Gastroesophageal Reflux
When stomach acid reaches the mouth, especially unnoticed reflux during sleep, it severely weakens teeth.
If reflux is not treated, dental treatments do not achieve full results.

Dry Mouth (Xerostomia)
Saliva is a natural buffering system; when reduced, the effect of acids increases.
Sjogren’s syndrome, certain medications, insufficient water intake, and prolonged talking/breathing habits contribute to this condition.

Excessive Acid Exposure (Occupational or Dietary)

  • Excessive citrus consumption
  • Fitness and high protein–acid load diets
  • Wine tasting
  • Wrong online habits like rubbing teeth with lemon

All of these weaken the enamel.

Symptoms of Tooth Erosion

Tooth erosion progresses silently. Patients usually describe the following symptoms:

  • Sudden sensitivity to cold, heat, and sweet foods
  • Yellowing of teeth (enamel thins and dentin becomes visible)
  • Softening and dull appearance of tooth surfaces
  • Small cracks and thinning
  • Flattened chewing surfaces
  • Discomfort or soreness while brushing

In erosive wear, teeth appear short and worn; smile harmony begins to be disrupted.

How Is the Diagnosis Made?

During the physician’s examination, enamel thickness, color changes, surface texture, and sensitivity are evaluated.
If necessary, intraoral photographs, micro-surface analysis, and wear maps are used.
Reflux and dietary habits are always questioned; in some cases, gastroenterology support is important.

Treatment of Tooth Erosion

The goal of treatment is to stop the loss, strengthen the existing structure, and preserve natural aesthetics.

Preventive and Supportive Approaches

  • Remineralizing gels and toothpastes
  • Fluoride varnish applications
  • Protocols to increase saliva flow
  • Products to reduce sensitivity

This approach is very effective in early cases.

Restorative Interventions (Advanced Cases)
If the enamel is significantly thinned, the tooth surface needs to be strengthened:

  • Microinvasive composite restorations
  • Overlay and veneer applications
  • Redesign of the chewing surface

The goal is to stabilize the structure while preserving natural tooth tissue.

Dietary and Lifestyle Adjustments

Behavioral adjustments are as important as treatment.
Protective strategies include consuming acidic drinks at once, using a straw, and avoiding immediate brushing after acid exposure, as brushing can further wear enamel softened by acid.

For reflux patients, nighttime oral care and medical follow-up are recommended; for athletes, controlling dry mouth; and for those who consume coffee/tea frequently, monitoring water intake — personalized recommendations should be provided.

Aesthetic and Functional Planning in Erosive Wear

Erosion affects not only sensitivity but, over time, lip support, facial height, and aesthetic lines as well.
Therefore, treatment sometimes requires not just fillings but comprehensive smile planning and chewing rehabilitation.

Intervening before the length and facial proportions of the teeth are lost prevents more radical procedures later.
Changes detected early make treatment both easier and result in a more natural outcome.

Protecting the enamel is not just “dental treatment”; it is the foundation of long-term oral health investment.
